Application of Criminal Record Certificate

By the request of the Ministry of Foreign Affairs, the police are issuing "Criminal Record Certificate", or often referred to as "Police Certificate", upon applications from those who are required by public institutions such as foreign embassies/consulates.
At the Okayama Police, applications can be made for those who currently has his/her resident registration or alien registration within Okayama prefecture, or those who has had his/her last resident registration or last alien registration within Okayama prefecture.

Necessary Documents
(1) Documents that state your reason of applying Criminal Record Certificate
* Digital documents should be printed out and bring it to apply.
(Please refer to the list of documents below.)
(2) Your valid original passport
(3) Documents that verify your current address.
(4) Processing fee:
750 yen is required.
* Cash only.
* Please have exact change ready.

〈The list of documents〉
Documents(1):
Documents that state your reason of applying Criminal Record Certificate (Any one of the following documents is necessary.)
a. Documents issued by foreign government that state obtaining Criminal Record Certificate are required.
b. Completed Visa application forms and others.
Documents(2):
Documents that verify your name and current address
Documents differ depends on applicant's resident status, please bring all pertinent document to apply.

* Japanese nationals living in Japan (Any one of the following documents is necessary.):
a. Driver’s license
b. An official copy of a Certificate of Residence(“Juminhyo”) (Issued within the last 6 months.):
c. Certificate of Items Stated in Resident Register (Issued within the last 6 months.):
d. My Number Card and so on

* Foreign nationals living in Japan (Any one of the following documents is necessary.):
a. Residence Card
b. Special Permanent Resident Certificate ("Tokubetsu Eijusha Shomeisho")
c.An official copy of Certificate of Residence("Juminhyo") (Issued within the last 6 months.)

* Japanese nationals living abroad (Two of the following documents are necessary.):
a. A document showing that you have moved out of Japan (Any one of the following documents is necessary.)
(a) An official copy of a Record of Deletion from Certification of Residence
(b) An official copy of a Family Register Attachment
b. A document that verify your current address
Valid driver's license issued by foreign government  and so on.

* Foreign nationals living abroad (Two of the following documents are necessary.):
a. A document that shows you lived in Okayama
An official copy of Alien Registration Certificate( Issued within the last 6 months.)
b. A document that verify your current address
Valid driver’s license issued by foreign government and so on.

Important notice: Please call Criminal Identification Section in advance to confirm necessary procedures. Your application may not be immediately accepted depending on the nations requesting a certificate of your criminal record or the reasons for application.
Others:
a. Your must apply in person.
b. It should take approximately 10 business days for processing.
c. Either you or your representative may receive the certificate ( your representative needs to bring a letter of proxy authorized by you, along with your representative's identification.)
d. If you are a Japanese national living in abroad, you can also apply to Japanese embassy or consulate of the country you reside. In case you have already made an application at a Japanese embassy or consulate, you cannot apply in Japan.

Directions:
a. 15 minutes' walk from JR Okayama Station to Scientific Criminal Identification Center.
b. Bus transportation:
Take Okaden-Bus lines from JR Okayama Station Bus Terminal, bound for "Kokuritsu-Byoin". Get off at "Kosenkyo-higashi" bus stop. It takes 3 minutes' walk.
If you are coming by car, please be careful of numerous one-way streets.
